5 Essentials Student Survey-The 5Essentials Survey asks students about their experiences and feelings, such as support from their teachers, how safe they feel at school, their school work, and plans for the future. Students in grades 4th-12th grade are invited to complete the survey. If you would like to opt-out your child from completing the survey, please complete this opt-out form no later than Thursday, March 17th at 3:00pm.
